RECURRENCE OF BREAST NEOPLASM IN MULTIPLE SCLEROSIS: CASE REPORT

RESUMO

CASE PRESENTATION: Female, 54 years old, with Relapsing-Remitting Multiple Sclerosis, diagnosed in February 2013, she used Betainferon 1a and is currently using Glatiramer Acetate. In 2017, she presented a breast nodule in the left breast, identified as Invasive Ductal Carcinoma, undergoing neoadjuvant treatment with chemotherapy and subsequent excision, which occurred without intercurrences or complications. Later, in 2022, she presented a nodule in the right breast, and a biopsy was performed and identified as Invasive Ductal Carcinoma, with estrogen and progesterone receptor positive and HER2 negative, being in clinical follow-up and therapeutic management with hormone therapy. DISCUSSION: Multiple Sclerosis (MS) is an autoimmune disease of a degenerative nature, which leads to demyelination of the Central Nervous System. Regarding the link with neoplasms, in a cohort study, it was found that the presence of MS is associated with an increase in all-cause mortality, but it was not associated with changes in neoplasm specific survival. In patients with MS, longer diagnostic intervals may occur if the symptoms of neoplasms were wrongly attributed to the underlying disease or if the cancer symptoms were less relevant than other health problems. In addition, MS related disability, which increases with age, as well as the occurrence of neoplasms, can make access to diagnostic tests, such as mammography, difficult. Thus, the lower rate of cancer detection through screening in these patients compromises disease control, negatively affecting survival. FINAL REMARKS: Studies show that patients with MS do not appear to have an increased risk of developing neoplasms. However, some have suggested an increased risk not only of breast cancer, but also of the CNS, urinary tract and nasopharynx. Commonly, these observations are based on a small number of cases, which makes these estimates inaccurate. Thus, understanding the pathways to early detection of breast cancer is critical to developing and planning interventions to improve outcomes for people with MS and cancer.
