Palliative Care in Patients Hospitalized for Stroke: An 8-Month Time Frame During the SARSCOV-2 Pandemic

Introduction: Since March 2020, Brazil has been experiencing the waves of the pandemic caused by the SARSCOV-2 virus. Stroke is an important cause of mortality and morbidity in the country and in the world, and therefore it is imperative to consider Palliative Care in cerebrovascular diseases. During the pandemic, it was reported that patients were afraid to look for emergencies with symptoms of stroke for fear of contracting COVID-19. Objective: To describe hospitalizations for stroke from October 2021 to May 2022; describe the cases of COVID-19 in these patients during hospitalization; describe the outcome of these hospitalized patients; describe epidemiological variables of patients (age and sex). Method: Cross-sectional, observational, quantitative study in patients admitted through the emergency department of a private hospital from October 2021 to May 2022. Inclusion criteria were patients hospitalized for ischemic, hemorrhagic stroke and Cerebral Venous Thrombosis (CVT). Patients hospitalized for other cerebrovascular syndromes, as well as those who had a stroke after hospitalization, were excluded. Data were collected from the patients' medical records and analyzed using Excel and SPSS programs. The study is part of the research project approved by the hospital's CEP -CAE 48835021.6.0000.5043. Results: 75 patients were studied, 60 of which were diagnosed with Ischemic Stroke, 14 with Hemorrhagic Stroke and 1 with CVT. The mean age of the patients was 75.8 years and 60% were female. 86.66% were discharged from hospital, 3 remained hospitalized until the end of the clipping and 7 of them died. Of all patients, 6.66% were diagnosed with COVID-19 during hospitalization. 11 patients were indicated for palliative care, and of these patients, 54.54% died, 18.18% were discharged from the hospital and 27.27% were hospitalized by the end of data collection. Conclusion: Most patients were hospitalized for ischemic stroke, aged over 70 years and were discharged from hospital. Of the 75 patients, 5 cases were found with a positive COVID-19 test and of the 7 deaths, 6 entered the hospital's palliative care line. The study is relevant in assessing the importance of the stroke itself, even in the face of the pandemic. The authors also emphasize the importance of Palliative Care for stroke patients. There is a need for further analysis on the subject, in order to approach the patient and his family in the most appropriate and comprehensive way possible.
