Cerebellar Subependymoma in an Adult Patient: Case Report

RESUMO

Case Presentation: A 29-year-old male patient underwent a biopsy of a cerebellar tumor. Grossly, the fragments obtained were irregular, brown and soft. Microscopically, circumscribed glial neoplasm was observed, formed by clustered nuclei, without atypia in the midst of occasional microcysts with fibrinoid content. No mitotic or necrotic figures were identified. The association between clinical aspects, macroscopic features and microscopic findings defined the diagnosis of subependymoma, a grade I tumor according to the 2021 World Health Organization (WHO) classification. Discussion: Subependymomas are rare, well-circumscribed, slow-growing and often asymptomatic masses with an ependymal origin that are most commonly found in the ventricular system, usually in the fourth or lateral ventricle. Its rarity is reflected in the scarce number of reports in the scientific literature. They represent 0.2 to 0.7% of all intracerebral tumors and can manifest with cerebrospinal fluid obstruction, causing increased intracranial pressure. The intraparenchymal site is very rare and other tumor locations include the septum pellucidum, foramen of Monro and, less commonly, the spinal cord. Most tumors are found incidentally at autopsy. Symptomatic subependymomas most often affect middle-aged patients and may rarely manifest with intratumoral hemorrhage or seizures. Surgical exploration and biopsy are necessary to distinguish the appropriate treatment. Usually, tumor excision and radiotherapy are strongly recommended and often corrective, with tumor recurrence being quite rare. Final Comments: Our case intends to register this rare case of subependymoma, recommend more studies to understand the possible origin of this tumor and highlight this possibility for the diagnostic range in clinical picture of dizziness, emphasizing the importance of histopathology in defining the best course of action.
