PREDICTORS OF HEMORRHAGIC TRANSFORMATION IN ISCHEMIC STROKE PATIENTS: EXPERIENCE OF A BRAZILIAN TERTIARY HOSPITAL

Resumo

Background: Hemorrhagic transformation (HT) is one of the most common stroke-related complications. Predictors of this complication are still unclear in the literature and understanding them may be a resource of unique clinical value to predict this dreaded condition. Objective: Identify predictive factors for HT in patients with ischemic stroke submitted or not to reperfusion therapies (RTs). Methods: We analyzed retrospectively data from all patients admitted to a South-Brazilian Stroke Center, from 2017 to 2019. Results: 821 patients were included in the analyzed sample. The prevalence of any HT was 9.7%. Thrombolysis with thrombectomy (n=12) was independently associated with TH (OR 4.7, 95% CI 1.4-16.1, p=0.013). Thrombolysis with tPA was not associated with TH (p=0.1), in contrast to performing thrombectomy alone (n=12/805, 1.5%). In general, TH was not related to a worse clinical outcome (mRS3-6) in adjusted analysis. Conclusion: The value of low ASPECTS score, high admission NIHSS, and cardioembolic etiology were possible predictors for HT in patients who did not undergo rt-PA thrombolysis.
