EPIDEMIOLOGICAL AND CLINICAL ASSESSMENT OF PACIENTS WITH UVEAL MELANOMA FROM TWO INSTITUTIONS IN THE STATE OF SAO PAULO AND ELABORATION OF MULTIDISCIPLINARY CARE PROTOCOL AT THE GENERAL HOSPITAL OF THE UNIVERSITY OF CAMPINAS

Introdução

The melanoma, although rare, deserves to be highlighted among ocular neoplasms because it is difficult to diagnose and confer high mortality. The clinical pathological aspects of patients with uveal melanoma (UM), the most common type of ocular melanoma (OM), are unknown in our state and country.

Objetivo

Still, there is no standard conduct among professionals who provide assistance to these patients at the General Hospital of the University of Campinas (UNICAMP).The objectives of the study were: 1) to identify the pathological clinical aspects of UM in patients
seen in two institutions on the state of São Paulo, 2) to analyze the rates of disease-free survival (DFS), progression-free survival (PFS) and overall survival (OS) of patients with UM and the influence of clinical pathological aspects on survival rates and, 3) establish a multidisciplinary care protocol for patients with UM at the General Hospital of UNICAMP.

Método

All patients with UM treated at the Ophthalmology and Clinical Oncology of the General Hospital of UNICAMP between 01/01/2000 to 12/31/2017 were retrospectively evaluated and those treated at the Hospital of Beneficencia Portuguesa of São Paulo between 01/01/2015 to 12/31/2019. The demographics aspects of the patients, the biological aspects of the tumor, the therapies administered, and survival times were obtained from patients' medical records. The flow of patients in the Institution, the therapy, and the support they received during the followup at the services were also considered.

Resultado

In total, 95 patients were included in the study, average age was 56,3 years, 50% were women and 67% were white .74% of the tumors were in the choroid, the average thickness was 10,5mm and the diameters was 15,9mm. Chemotherapy was administered to 45 patients and immunotherapy to 13 patients. Patients from UNICAMP had lower OS than Hospital Beneficencia Portuguesa de São Paulo (P=0,001.

Conclusão

The clinical and biological characteristics (age, sex, skin color, tumor location in the eye) of our patients with uveal melanoma were like those of patients from other parts of the world. Age and thickness of the tumor also altered the survival of patients with UM in other parts of the world. The higher rate of DFS in the General Hospital of UNICAMP and the higher rate of OS in the Hospital Beneficencia Portuguesa de São Paulo may be the result of different patient characteristics, different types of local treatment performed and follow-up with different imaging exams in both institutions.
