IMPLEMENTATION OF A PAIN MANAGEMENT SERVICE IN AN OPEN CLINICAL STAFF HOSPITAL: STRATEGIES AND RESULTS.
Introdução
Pain Clinic Services in the Oncology Sector have the primary objective of caring for cancer patients, structuring itself in the concept of total pain, integrating physical, emotional, social and spiritual domains, harmonizing the care team, signaling a painful event for proper diagnosis and treatment. The implementation of these services has become frequent, justified by the improvement of individualized care provided, achieving pain control, preventing and/or minimizing adverse effects of medications used, avoiding the suffering of patients.
Objetivo
This paper aims to identify, describe and share the results obtained from the follow-up by the specialized pain team in the treatment of cancer patients hospitalized in a private and open hospital in Rio de Janeiro.
Método
This is a descriptive report of the results obtained by the Oncology Pain Clinic Service, from June 2019 to June 2021, with the follow-up of 86 patients, identified by the automatic triggering criteria. The following data selected by the medical team and used by the nursing team for contact are considered criteria: patient request, one episode of pain intensity ≥ 7, three episodes of pain intensity ≥ 4, pain interrupting sleep, pain interrupting activities (physiotherapy), side effects medication, presence of epidural, subarachnoid or peripheral nerve catheter (with or without infusion pump) and implanted devices (intrathecal pump or neurostimulator).
Resultado
The actions of the specialized team represented significant clinical improvements in the 86 patients followed: 80% reduction in pain intensity by 96% (83/86), control of nausea and vomiting by 95% (82/86) and reduction in constipation in 93% (80/86). The most prevalent pain was the mixed type (81/86), the most painful metastasis was osseous metastatic disease (72/86).
Conclusão
Pain treatment performed by a specialized professional involves the implementation of an effective individualized analgesic regimen with no or minimal side effects. In order to be a successful and reproducible model, early intervention is necessary, through the constant presence of the specialized team, continuous multiprofessional education and the dissemination of triggering criteria carried out by the qualified nursing staff.
