Analysis of prognostic factors in surgical treatment of gastric cancer
Introdução
Gastric cancer is one of the most frequent neoplasms. New prognostic factors are needed to better determine the evolution of patients.
Objetivo
To evaluate the impact of major prognostic factors of gastric adenocarcinoma in overall survival of patients undergoing surgical treatment of neoplasia at the Evangélico Mackenzie Universitary Hospital in Curitiba (HUEM)
Método
: Retrospective analysis of epidemiological, clinical and pathological findings of 43 patients by the oncology service HUEM between the years 2005 to 2014.
Resultados
The average patient age at diagnosis was 60/63 years old, a higher prevalence in male patients (72.1%). All had some type of symptom. Sample the majority of patients were nonsmokers (72.1%) and / or alcohol drinkers (81.4%). Total gastrectomy (58.1%) and subtotal (41.9%) were the procedures performed. The average number of lymph nodes removed was 13.56. No patient underwent D3 lymphadenectomy. Most patients (55.8%) underwent adjuvant chemotherapy due to locally advanced or metastatic stage (60.5%). Only 1 patient (2.3%) was submitted to therapy combined with radiation therapy. The overall survival (OS) in patients in two years was 46.2%. With regard to TNM staging, patients with stage III and IV had a higher risk of death approximately three times higher and lower survival rates (32.2%) compared to patients stage I and II (66.7%).
Conclusão
TNM staging remains the main determinant of evolution and survival of patients with gastric adenocarcinoma.
