Título

Hospital morbidity and mortality from colorectal cancer in times of the Covid-19 pandemic: what changed between 2019 and 2020?

Introdução

Colorectal cancer is a frequent and lethal disease that affects part of the large intestine, colon, and rectum. In Brazil, there was a morbimortality increase in the last decade, likely from various factors, such as better accuracy of diagnosis, due to technological advances. Due to the COVID-19 pandemic, hospitalizations decreased in Brazil in 2020, going from 5.8 to 4.4 hospitalizations per thousand inhabitants compared to 2019. Digestive diseases were the third chief cause of these hospitalizations in the same year, emphasizing the importance of evaluating the pandemic impact on the morbidity and mortality of colorectal cancer.

Objetivo

Characterize the epidemiology of hospitalizations for colorectal cancer during the months of the pandemic in 2020, compared to 2019.

Método

Ecological study using hospitalization data for colorectal cancer hospitalizations in Brazil in 2019 and 2020, from the Database of the Information Technology Department of the Unified Health System (DATASUS). The epidemiology of admissions during the pandemic year was analyzed, comparing it with the previous year, addressing region, age group, gender, average hospital stay, and mortality rate.

Resultado

With the pandemic, there was a reduction of 10.97% in hospitalizations for colorectal cancer, from 52,791 in 2019 to 46,999 cases in 2020. Despite a drop of 2.18% compared to the pre-pandemic, the Southeast region remained with the highest cases. There were no significant differences in the periods analyzed regarding the most affected age group, from 50 to 79 years old, increasing from 73.72% in 2019 to 73.27% in 2020 and gender, with the female population growing 0.04% from 2019 to 2020. The average hospital stay decreased from 5.1 days to 4.7 days in 2020. Moreover, there was an increase in the mortality rate, which went from 7.96% to 8.09% during the pandemic year.

Conclusão

In conclusion, there was a reduction in hospitalizations for colorectal cancer, possibly explained by the reallocation of hospital beds to patients diagnosed with COVID-19; this may also explain the increase in mortality caused by the disease, perhaps due to less medical follow-up.
